and print the WPGS Publications Order Form along with a check
made out to Western Pennsylvania Genealogical Society for the cost of the books
plus shipping/handling and tax. For continental USA addresses only, the
shipping/handling is $4.00 for the first item and $1.00 for each additional item
on the same order. There is one exception. When ordering the Pittsburgh Gazette
Abstracts, 1786-1820 which includes five volumes, purchasers must include $9
shipping and handling for that set of books. Pennsylvania residents please add 7% sales tax. For shipments

The Superintendent of every coal mine in Pennsylvania was required to send to the
Mine Inspector an annual report listing fatal accidents occurring within their
district for the previous year. This volume includes a summary of those
published reports for the western bituminous counties of Pennsylvania for the
years 1888 through 1897. Entries include the name of the deceased, date of
accident, date of death, location of accident, his occupation, and age. Most
